This Form Privacy Notice explains the appointment request and contact controls displayed on the Capital Foot Care website. It describes their current behavior, the purpose of the field labels, and the available way to contact the practice. Read it together with our website Privacy Policy.
Current form controls
The appointment request and contact controls are inactive. Their fields are disabled, and the Submit Request and Send Message buttons cannot submit information. The controls do not send a name, telephone number, email address, date preference, location choice or message to the practice.
These fields do not create stored contact requests, appointment requests or patient records. A visible field label or button does not mean a communication has taken place. For appointment arrangements or questions about your visit, call Capital Foot Care at 301-927-3668.
Appointment field labels
The appointment layout includes Full Name, Phone Number, Email Address, Preferred Date, Preferred Time and Preferred Location. These labels identify the types of details associated with an appointment request. The date and time labels describe preferences; they are not a list of available appointments.
The location options identify the four offices shown on our Locations page. An office name in that list does not establish that a particular provider or procedure is available there. Use the office phone number to discuss the visit relevant to you.
Contact field labels
The contact layout includes name, phone, email, subject and message fields. The text shown inside an empty control identifies the field's purpose. It is not a patient's name, contact detail or submitted message.
The website forms have no document attachment, photograph upload, medical-record upload or payment feature. The disabled fields cannot deliver an accessibility report, a question about the office or a description of a medical concern. Use the practice phone number for direct contact.
What website links do
A Request an Appointment link opens the appointment page. A Contact Us link opens the page with the practice phone number and office details. Following one of these links does not transmit the form fields or send a message.
Telephone links can open your device's calling function. Directions links open Google Maps, and the Facebook link opens a separate social website. Those services have their own functions and privacy practices. Their use is separate from the inactive controls on the Capital Foot Care website.
Medical information and records
The website controls are not a patient-record portal or a channel for sending health records. If you want to provide documents or photographs to the practice, call the office to ask how to do so. Do not rely on an inactive website field to communicate that information.
This notice addresses website controls. It does not replace an official Notice of Privacy Practices about medical information or describe the handling of records created during care. Questions about medical-record privacy should be directed to the practice.
Your browser and device
Your browser may offer its own settings for saved contact details, history, downloads and permissions. Those settings belong to your browser or device. The website's disabled controls do not accept submitted details from those features.
You can read the service pages, provider profiles and education articles without completing a form or signing in. Our Privacy Policy explains other website features, including browser navigation and links to public documents and external services.
